Please use this form to submit a Travel League Roster.
Required fields are marked with an *. At least one player must be entered.
This form can be used to add additional players to an existing roster. You must ensure the Team Name is an exact match to the Team Name on the existing roster.
The form will not be submitted until all required fields are entered. When Submit is selected, captions of required fields not entered are changed to a bold red font to identify the missing information.
The information entered on this form is emailed to the OHSBL to create your official Travel Team roster. The officla roster will be created as a PDF file that you can download and print from the link on the Travel League main page.
